I confirm and warrant that the contents of my application are true and accurate in every respect. I hereby indemnify the Irish Commercial Mediation Association in respect of any claims howsoever arising out of reliance upon the contents of this application or the Directory of Mediators and, in particular, in the event of a claim being brought by party arising out of the conduct or any other aspect of a mediation itself. I confirm that the conduct of any mediation to which I am appointed is entirely my own responsibility. I further confirm and understand that ICMA is not approving, recommending or otherwise howsoever validating either me or any of the details that I provide in connection with my application, including my aforementioned qualifications or training (or the entities from whom I have received same), and that any inclusion in the Directory of Mediators of some or all of the details that I provide in connection with my application is entirely at my own risk.

The personal information provided by me shall be processed by ICMA in accordance with the Data Protection Acts 1988 and 2003. If my application is successful, some or all of the personal details that I provide in connection with my application (and/or full details of or a copy of the completed form in its entirety) will be included in the Directory of Mediators. This Directory of Mediators may be published by ICMA in electronic (including publication on the ICMA website), CD rom and/or paper format. My personal information will also be stored on a database for administration purposes in connection with the publication and maintenance of the Directory and in order to provide updates and information. My personal information will not be used for any other purposes, save with my consent. It is up to me to notify ICMA of any change in my personal details. I can do this by contacting the Honorary Secretary of the Irish Commercial Mediation Association c/o 25-28 North Wall Quay, IFSC, Dublin 1. If my application is unsuccessful, the personal details I have provided will be destroyed.

  I hereby consent to the processing of my personal information for the purposes set out above.

  I confirm that I have read and shall adhere to the ICMA Rules of Membership

  and I confirm that I have read and shall adhere to the European Code of Conduct for Mediators.

  I confirm that I have the appropriate Insurance in place for practicing as a Mediator.
